The drug is metabolized by hepatic cytochrome P450 microsomal enzymes, which results in the formation of the active metabolites phosphoramide mustard and acrolein. It is the acrolein which is responsible for hemorrhagic cystitis as a complication of cyclophosphamide therapy. An early diagnosis and prompt management holds the key in its management.
